One time, I was having sex, legs over my head, my partner was fucking me hard, and I just kept moving up, further and further until my head was situated not very comfortably against the headboard. A few more thrusts and my head kind of slipped through the slats. Next thrust and we heard a huge POP! My shoulder got dislocated! That was not a fun ER visit!
So my most recent time was about 6 months ago I was working with a younger guy new to bondage. He started showing up at munchs and had interest in suspension play so I decided to try it with him and when I was in the air in a swan dive style tie (tied to body not body mod piercing style). something he did didn't go right though and I fell it gave me the sharpest pain in my side and I couldn't get up he cut all the ropes and tried to help me up but I couldn't stand. We had to call a ambulance and they took me to the hospital I ended up with flank pain. I had to stay at the hospital for 3 days and had to be out of work for over a month. My room mate even had to take of work for a few weeks since it was so bad I couldn't get up on my own so he had to do things like carry me to the bathroom.

So another time I had to go to the ER we were doing a scene with wax one of the few times I ever did anything with it I was tied up and my partner was dripping it on me and I guess some type of beauty product I had on was really flammable and my arm caught on fire and we panicked my partner got the fire out and cut my restraints off but we were both still kind of shaky and he ended up cutting my with the knife he used to cut the restraint and it was bad enough to need stitches.

I have also burnt a nipple with wax play. Thankfully not bad enough for the ER but enough to cause me to do a lot more research before we tried it again
